Carnitine may be useful for preventing
and/or treating many conditions. A list is pre-
sented in Dr. Alan Gaby’s book, Nutritional
Medicine (see table).
“Although carnitine is synthesized by the
body, there are a number of circumstances in
which the amount synthesized is inadequate
to meet physiological requirements. Primary
carnitine deficiency is a rare condition that
results from inborn errors of metabolism . . .
Secondary carnitine deficiency is more com-
mon, and may occur in association with vari-
ous diseases and clinical situations, including
burns, infection, surgery, starvation, con-
sumption of a ketogenic diet, malabsorption,
use of certain medications, renal tubular dis-
orders, dialysis treatment, liver dysfunction,
and infancy (particularly with prematurity).
Secondary carnitine deficiency usually results
from increased urinary carnitine excretion,
decreased synthesis, or a combination of
these factors . . .”
Manifestations of secondary carnitine defi-
ciency may include fatigue; muscle weak-
ness; lipid accumulation in skeletal muscle,
myocardium, and liver; and hypoglycemia. .

. Propionyl-L-carnitine is a carnitine deriva-

muscle. Acetyl-L-carnitine . . . is another car-
nitine derivative that functions as a choliner-
gic neurotransmitter and can also serve as a
source of L-carnitine. The clinical indications
for L-carnitine and acetyl-L-carnitine differ,
and these compounds are not used inter-
changeably.
Dosage and administration: The most fre-
quently used dosages of L-carnitine for adults
in clinical trials have been 1–2 g/day,
although lower and higher dosages have
been used in some studies. . . Taking L-carni-
tine in divided doses, as opposed to once a
day, may increase the total amount absorbed,
particularly at the higher end of the dosage
range.”
l-Carnitine is an amino acid found abundantly in skeletal
and heart muscle. It functions primarily to support fat utiliza-
tion, and also acts as a carrier of fatty acids into the mito-
chondria, where they are oxidized and converted to energy.
l-Carnitine also facilitates the removal of short and medium
chain fatty acids from the mitochondria that accumulate
during normal metabolic processes. A randomized double
blind, placebo-controlled trial of l-carnitine involving 101
volunteers indicated that l-carnitine maintained healthy
cardiac enzymes including lipid peroxides and lactate
dehydrogenase. In combination with other cardiovascular
support agents, l-carnitine supported ventricular function
and superoxide dismutase enzyme activity in human volun-
teers. Long-term l-carnitine supplementation indicated pos-
itive support for cardiovascular health after a 3-year follow-
up in adult subjects. A recent placebo-controlled clinical
study conducted at Ball State University in Indiana has
found that l-carnitine-l-tartrate supplementation helps physi-
cal recovery from high intensity exercise. l-Carnitine-l-tartrate
contains 68% pure l-carnitine and 32% natural l-tartaric acid. -Carnitine-l-tar-
trate is a highly stabilized form of l-carnitine supporting cardiovascular health
and recovery from exercise. -
