Today, I thought we would start
off by answering some questions
that were sent to me by email, via
my blog. I want to also comment
on some recent items in the news,
such as the new recommendations
on PSA testing. And, I want to talk
in more detail about some topics
we have commented on in recent
weeks, such as eye drops that show
promise in preventing cataracts and
other eye problems.
How many times over the years
have we had men call in, asking
the question, how do I lower my
PSA test result? Even after I careful-
ly explain to them that the PSA
reading is merely an indicator that
there might be an underlying prob-
lem with the prostate, perhaps
indicating early prostate cancer,
they continue to focus on the PSA
reading itself. They cannot let go of
the idea that the PSA reading is the
disease, and that lowering the PSA
reading will somehow cure the dis-
ease. They refuse to understand
that all the elevated PSA really
means is that they need to now
have their doctor look more closely
at their prostate, and determine if
there is or is not an underlying
problem. If there is a problem with
the prostate, be in inflammation,
enlargement or cancer, that is what
needs to be treated–not the PSA
reading. So I explain all of this, and
they say, "ok, thanks, now what can
I take to lower my PSA reading?"
Well, now that the latest expert
panel has chimed in, I have a new
answer to the question "how do I
lower my PSA test result?" Don't
take the PSA test!
The expert panel has advised that
most older men should not get rou-
tine screening for prostate cancer.
The U.S. Preventive Services Task
Force said that prostate-specific
antigen (PSA) screening does more
harm than good. A high test read-
ing can indicate prostate cancer.
But PSA can go up for other rea-
sons. Only a biopsy can tell if can-
cer exists. Some prostate cancers
can be deadly. But most grow so
slowly they never cause harm. Men
who get treated may have prob-
lems with sexual function and urine
control. And the panel concluded
that there's little or no evidence
that routine PSA tests save lives.
If your PSA level is elevated, it
can signal hidden prostate cancer.
But PSA also rises for many other
reasons. These include:
•An enlarged prostate
•A prostate infection
•After having sex or riding a bic
cle
Even when prostate cancer caus
es a rise in PSA, most often that
cancer would have otherwise gon
unnoticed. Only a minority of
prostate cancers harm a man's
quality of life or shorten how long
he lives.
Now, obviously, this panel's
advice is not the final word.
Whether or not you should contin
ue having PSA tests is a question
you need to decide after talking t
your doctor and evaluating your
personal situation.
