what the researchers did. Here is the rest of
the story.
The most common type of kidney stone is
one made up primarily of calcium and
oxalate. It might be called a "calcium stone,"
but the key component is the oxalate, not
the calcium.
"The relationship between dietary calcium
and kidney stones is complex. It is known--
and this study confirms--that increasing
amounts of dietary calcium are associated
with fewer kidney stones. It is believed that
this occurs because the increased amounts of
calcium from food binds to oxalate from
food in the intestine, and the complex of cal-
cium and oxalate cannot be absorbed into
the body. This reduces the amount of calci-
um and oxalate that needs to be eliminated
from the body in the urine and prevents the
formation of calcium/oxalate stones in the
kidneys. In fact, calcium supplements have
been used successfully to decrease the elimi-
nation of oxalate in the urine."
Don: Note what the investigators just said.
Calcium supplements have been shown to
reduce the amount of oxalate in the urine.
So you would expect that calcium supple-
ments would decrease the incidence of
stones the same way dietary calcium did,
wouldn't you?
Why then, were calcium supplements asso-
ciated with more kidney stones in the nurses?
Arnie: The study's investigators hypothesize
that the reason is the time at which the nurs-
es took the calcium supplements.
The calcium and oxalate in food can bind
to each other only if they are present at the
same time in the intestine. If calcium supple-
ments are taken with meals containing the
most oxalate, everything is fine, and the cal-
cium and oxalate bind. If calcium supple-
ments are taken with meals that contain little
or no oxalate, the oxalate doesn't bind to cal-
cium, and the oxalate is absorbed from the
intestine and then eliminated in the urine.
Furthermore, if calcium supplements are
not taken with food, more calcium is
absorbed and eliminated in urine. (This
would increase the risk for developing
stones.)
Don: Well, sure enough. It turns out that
57% of the nurses in the study taking supple-
ments were not taking them with meals, and
24% were taking the supplements only with
breakfast, the meal least likely to contain
oxalate.
Thus, more than three-quarters of the
nurses taking calcium supplements may have
been absorbing and eliminating more calci-
um in their urine or were not gaining the
benefit of the binding of calcium and oxalate
in the intestine.
Arnie: So what is the proper conclusions to
be drawn from this study? Is it that you
should not take calcium supplements
because they increase your risk of kidney
stone? No, not at all. The conclusion is that
calcium reduces the risk of kidney stones,
and that it's important to take calcium sup-
plements in the right way–with food.
Don: In a way, this just confirms what we
have been saying about calcium supplements
all along, whether related to kidney stones or
soft tissue calcification–for the body to prop-
erly handle calcium, it needs to be taken
with the proper cofactors–vitamin D, vitamin
K, Magnesium, trace minerals, etc. Taking
calcium by itself, especially in high dosages,
can indeed lead to undesirable effects.
With kidney stone risk, we know that. We
also know that there are many other things
that can be done to reduce risk, and if you
are someone who falls in that high risk cate-
gory, as defined byArnie a few minutes ago,
you should certainly be aware of these meas-
ures, and follow them. But that does not
mean you should avoid calcium supple-
ments. Instead, you should consider increas-
ing supplemental magnesium, using calcium
in the citrate form, etc.
Arnie: I'm going to summarize this by quot-
ing from the textbook, Nutritional Medicine,
by Dr. Alan Gaby:
"One might reasonably conclude from the
available evidence that consumption of a
high-calcium diet decreases the risk of kidney
stones; that taking calcium supplements
between meals may increase the risk; and
that the effect of taking calcium supplements
with meals varies with the oxalate content of
the meal, and may range from protective to
neutral to harmful. Because magnesium
appears to prevent stone formation, it is like-
ly that any increased risk associated with cal-
cium supplementation would be mitigated or
negated by combining it with magnesium. In
addition, any increased risk associated with
calcium supplementation might be mini-
mized by taking the calcium in the form of
calcium citrate, which has been shown to
increase urinary excretion of citrate (an
inhibitor of stone formation)."
