very important medical treatment. And they
act as if the effect of supplements like fish oil
is so great, so significant, that this creates a
problem. They caution against taking numer-
ous supplements because they fear a possible
cumulative effect, an interaction between
coumadin and the supplement.
Again, we have a problem with perspec-
tive. I have suggested, many times in the
past, that it should be looked at in just the
opposite way. If these supplements are really
that effective as anticoagulants, take advan-
tage of the fact. Adjust the dosage of
coumadin accordingly. Turn the supposed
negative into a positive. Does this make
sense?
Arnie: Well, here is an example of doing
just that. You all might remember the prob-
lem with grapefruit juice. It turns out that
grapefruit juice can interfere with enzymes
that break down certain drugs, thus prevent-
ing the drug from being metabolized normal-
ly. In effect, it retards the breakdown, and
the drug ends up being more potent than
expected.
Is this a good thing, or a bad thing? It
depends on your perspective. In general, it
was presented as a bad thing. It was another
scary example of the "dangers of natural sub-
stances."
Don: But why couldn't it be just the oppo-
site? Why couldn't it be a good thing? . . . a
way of enhancing the activity of certain
drugs?
Arnie: Well, that's exactly what researchers
at the University of Chicago Medicine won-
dered as well. They wondered if this–the use
of grapefruit juice–might be an effective way
to lower the dose of certain dangerous, toxic
drugs. In this case, they looked at anti-cancer
drugs.
The clinical trial – published in Clinical
Cancer Research – revealed that an eight
ounce glass of grapefruit juice every day
could offer cancer patients the same benefits
from an anti-cancer drug as they would get
from more than three times as much of the
drug by itself. It accomplished this by block-
ing the breakdown (metabolism) of the drug
in by the body.
These results suggest that a combination of
grapefruit juice plus cancer drugs could help
patients avoid side effects associated with
high doses, in addition to reducing the cost
of the medication.
Led by Dr Ezra Cohen, the research team
show consumption of grapefruit juice can
slow the body's metabolism of sirolimus – a
drug that is approved for transplant patients,
but may also help those with cancer.
"Grapefruit juice, and drugs with a similar
mechanism, can significantly increase blood
levels of many drugs," explained Cohen, "but
this has long been considered an overdose
hazard.
"We wanted to see if grapefruit juice can
be used in a controlled fashion to increase
the availability and efficacy of sirolimus," he
said.
The team reveal that cancer patients who
drank grapefruit juice increased sirolimus lev-
els by 350%.
"This is the first cancer study to harness this
drug-food interaction," the authors note.
Don: So, again, it is a matter of perspec-
tive. Let's not let ourselves be misled. The
fact that a supplement exerts the same bene-
ficial effect as a drug may inconvenience the
physician, necessitating dosage adjustments,
etc, but that does not mean we should be
warned away from the supplement.
Listen carefully to what the researchers
involved in the grapefruit study point out,
because thereis another factor involved as
well–dollars.
Arnie: Here is what they said: "Therefore,
we have at our disposal an agent that can
markedly increase bioavailability (in this
study by approximately 350%) and, critically
in the current environment, decrease pre-
scription drug spending."
That's interesting. Maybe "perspective" if
also influenced at times by money?
Listen to this. The team also highlighted
that they received support from the US
National Institutes of Health for the study –
and not from any pharmaceutical company –
saying that dose-finding studies are "not nec-
essarily profitable" for pharmaceutical com-
panies; particularly, they add, if the study
outcome suggests lower doses than those rec-
ommended by the manufacturer after the
drug has been approved and priced.
Don: We get so many calls from people
who are worried that they might have a
problem taking supplements because of pos-
sible interactions with drugs. Usually the con-
cern is overblown. It's not necessary your
fault. I blame the doctors and nutritionists
who exaggerate in an attempt to support
their anti-supplement bias. Yes, many supple-
ments have anti-coagulant actions. But it is
usually so small it's insignificant. Even fish oil
has been shown to be significantly less of a
concern than many make it out to be. Read

supplements that can be so beneficial for the
wrong reasons. Fish oil is a good example. It
